Rebecca Sudore and Team Developed New Advance Care Planning Guide
Dr. Rebecca Sudore was part of a collaborative team of organizations, lawyers, and medical experts from the ABA Commission on Law and Aging, the American Academy of Hospice and Palliative Medicine, the University of California San Francisco Medical School and the UC/Hastings Consortium on Law, Science & Health Policy that developed a new advance care planning guide called the “Advance Directives: Counseling Guide for Lawyers.”
The guide includes:
- eight key advance care planning principles for clinicians and lawyers to better align practices in the legal and health care setting
- a checklist that includes key steps to walk through during lawyer/client advance care planning meetings
- leading advance care planning resources, including PREPARE For Your Care.
