Welcome Jacqueline Carranza, Dementia Care Aware Patient Navigator!
Please join us in welcoming Jacqueline Carranza, Dementia Care Aware Patient Navigator, to the UCSF Division of Geriatrics!
Jacqueline started this morning and will be supporting the Dementia Care Aware (DCA) San Francisco Health Network’s (SFHN) primary care clinics. Jacqueline earned her Bachelor of Science degree in Criminal Justice from San Francisco State University (SFSU). She also has a certificate for HIV/HCV Testing & Counseling Skills plus a Community Mental Health Worker Certificate.
In her most recent role as Administrative Officer at UCSF Adult Cardiothoracic Surgery, she worked with four Cardiothoracic surgeons supporting complex calendar scheduling and operational tasks. She also worked closely with the Education Program Administrator in coordinating CME accredited Thoracic Tumor Board and Adult Cardiothoracic Surgery Grand Rounds. Prior to this role, she was an Administrative Assistant at UCSF Alliance Health Project. She was the first point of contact in communicating with the crisis and Triage unit for individuals in distress. Jacqueline is fluent in Spanish and provided translations for Spanish-speaking patients to ensure quality of care.
Jacqueline has over five years of experience working in the Mental Health field. She has a wealth of experience in supporting clinical care teams from providing patient triage to psychiatry services.
In Jacqueline’s new role as Patient Navigator, she will work closely with the DCA Implementation Team to support the design and implementation of program interventions for a client population of adults with dementia and their care partners.
